Response required within 40 days

If you have TennCare, CHOICES, QMB or SLMB, you may receive a renewal packet in the mail. Responses are requested within 40 calendar days. If you have received your renewal packet and have not responded within 40 days, it is important that you respond as soon as possible; you may be eligible to keep your coverage while the renewal packet is being reviewed. If you no longer have the renewal packet, call Tennessee Health Connection (TNHC) at 855-259-0701 to request one.

Please fax responses to TNHC at 877-430-0843 or mail to TNHC, P.O. Box 20201, Nashville, TNĀ  37202-0201.

For renewal packet questions or general program assistance, call the State Health Insurance Assistance Program (SHIP) at 931-432-4150.
